cryptpad.fr has one, but it wouldn’t load on a friend’s phone, and this poll must absolutely work for everyone.

framaforms.org is only in French?

liberaforms.org only allows 250 answers/year/account, and this is a form for ~100 people.

opnform.com looks good but isn’t hosted in the EU.

I’m leaning toward this approach to create flexible forms like NewUserForm for different purposes. For example:

- email, zip code, address, …
